Primary Care Nutrition Pathways

The following care pathways developed by the Primary Care CMTF working group are evidence and consensus-based. An environmental scan identified key components and stakeholder engagement at a face-to-face knowledge exchange meeting of diverse providers from community and hospital resulted in initial versions. These early versions underwent review and rating of feasibility and acceptability by over 250 providers from across Canada.  Guidance documents have been created to support these pathways.
