The following questionnaires, surveys and audit tools will help you to engage teams and promote change. These tools have been used in research but are also designed to be used in your practice to identify where change is needed, as well to evaluate an intervention.
These indicators are focused on patient safety and are considered mandatory for ensuring adequate nutrition care is provided. CMTF recommends that hospitals begin to collect data using these indicators and as INPAC becomes more embedded in daily practice the optional indicators can be added. It is important to realize that the numbers will be low to start but as practice changes the numbers will improve.
This audit tool can be used to monitor the implementation of the key components of the Integrated Nutrition Pathway for Acute Care (INPAC).
Staff Survey - Malnutrition Knowledge, Attitudes and Practices
