Auditing Practice

INPAC & the INPAC Toolkit

The following questionnaires, surveys and audit tools will help you to engage teams and promote change. These tools have been used in research but are also designed to be used in your practice to identify where change is needed, as well to evaluate an intervention.

Indicators
These indicators are focused on patient safety and are considered mandatory for ensuring adequate nutrition care is provided.  CMTF recommends that hospitals begin to collect data using these indicators and as INPAC becomes more embedded in daily practice the optional indicators can be added.  It is important to realize that the numbers will be low to start but as practice changes the numbers will improve.

INPAC Audit
This audit tool can be used to monitor the implementation of the key components of the Integrated Nutrition Pathway for Acute Care (INPAC).

Staff Survey - Malnutrition Knowledge, Attitudes and Practices
This questionnaire is designed to evaluate the level and change in knowledge, attitudes and practice of hospital staff, before and after implementation efforts. You can also compare your results to those of the More-2-Eat hospitals here.

Mealtime Audit Tool: Guidance Document
This document explains how to complete the Mealtime Audit Tool.

Mealtime Audit Tool
This tool will track general observations of the unit and identify specific challenges or barriers to food intake as experienced by the patient.

Patient Mealtime Satisfaction Survey for Non-selective Menus
This validated survey was used in the Nutrition Care in Canadian Hospitals study and yielded very informative results.  This version is to be used in hospitals that have non-selective menus.

Patient Mealtime Satisfaction Survey for Selective Menus
This validated survey was used in the Nutrition Care in Canadian Hospitals study and yielded very informative results. This version is to be used in hospitals that have selective menus.

Nurse Survey
The following survey was used to determine nurses’ level of knowledge, attitudes and practices with respect to nutrition care of hospitalized patients

Physician Survey
The following survey was used in the CMTF study to determine physicians’ level of knowledge, attitudes and practices with respect to nutrition care of hospitalized patients
