Collaboration contre la malnutrition dans les soins primaires

Communauté de pratique 2021-2025 

En septembre 2021, le Comité de réflexion sur les soins primaires du Groupe de travail canadien sur la malnutrition (GTCM) a mis sur pied une communauté de pratique, appelée Collaboration contre la malnutrition dans les soins primaires. Cette initiative représentait une occasion unique pour les diététistes travaillant en soins primaires et en milieu extrahospitalier d’échanger sur des centres d’intérêt, des motifs de préoccupation et des défis communs, mais aussi de partager des ressources et des leçons. Les professionnels de la santé qui le souhaitaient pouvaient s’inscrire à la communauté de pratique pour apprendre, interagir et discuter.

De septembre 2021 à juin 2025, dans le cadre de 20 séances de mentorat de la communauté de pratique, 735 diététistes et autres professionnels de la santé ont réfléchi à la façon d’adopter des pratiques exemplaires en matière de dépistage, d’évaluation, de traitement et de prévention de la malnutrition chez les adultes. Les membres de la communauté de pratique avaient aussi accès aux enregistrements archivés des séances de mentorat. La liste des séances antérieures figure ci-dessous.

Dès septembre 2025, il n’y aura plus de séances de mentorat de la communauté de pratique Collaboration contre la malnutrition dans les soins primaires. Pour toute question, veuillez écrire à info@cns-scn.ca.
