Primary Care Malnutrition Collaborative:
A Community of Practice 2021-2025
Starting in September 2021, the CMTF Primary Care Working Group created a unique opportunity for dietitians working in primary care and the community to share common interests, concerns, challenges, resources and learnings. The Primary Care Malnutrition Collaborative was created to enable health care professionals to sign up to CMTF's Community of Practice to Learn, Engage and Discuss.
From Sept 2021 to June 2025, through 20 Community of Practice mentoring sessions, 735 dietitians and other health care professionals brainstormed on how to adopt best practices related to screening, assessing, treating and preventing malnutrition in their adult patients. Those who joined the collaborative also received access to archived recordings from the mentoring sessions. See list of past sessions below.
As of September 2025, the Primary Care Malnutrition Collaborative Community of Practice mentoring sessions will no longer be offered. For questions, contact info@cns-scn.ca
